Please note, SportsWareOnLine DOES NOT support the use of a signature field (the pink tab) within the Attachments tab. Make sure all fields are text fields that can be typed into. Within SportsWare there is a separate checkbox and text box for athletes to sign/authorize their filled documents. This allows the software to sort documents appropriately.
Using Free Tools
Below are two methods using free tools to create a fillable PDF form. Choose whichever method you prefer.
Using Adobe Acrobat Pro (Paid)
Please make a document within Microsoft Word or a similar text editor. Using the shape tool to make boxes or lines will allow Adobe to find the appropriate fields to make fillable. After you have your document created and saved as a PDF (instead of a docx) you can follow the below tutorial.
